Head Start Registration

Registration is open to all children who are 2.9 to 4.11-years-old, regardless of race, color, gender, creed, national origin or disability. Applicants must meet the Federal Poverty Guidelines. Bank Street Head Start is open Monday through Friday from 8:00 a.m. to 4:00 p.m.

What to bring to register:

  • Certified Birth Certificate
  • Verification of Income
  • Immunization Health Card
  • Two Proofs of Address
  • Child’s Medical Insurance Card (if available)
  • Completed Application* (PDF)

*If you are able to download the attached Application please complete it and bring it with you to your application appointment. If you cannot print out the application it can be filled out at your appointment.
