Registration and Medical Forms
There are two portions to registering.
First, you must fill out the Preliminary Registration page, where you will enter some key information about the camper as well as pay a deposit of $200 per session and an non-refundable processing fee of $40.
Second, you must fill out ACA Health and Camper Information Form #1. You can fill out the immunization portion yourself or you can have a doctor email us at bankstreetsummercamp@bankstreet.edu or fax us that information at 212.665.2811. This may be filled in by parent/guardian or a copy of immunization forms from health-care providers or state or local government are acceptable; please fax or email forms within 10 days of registration. Children without immunizations and records will not be accepted into the camp. Provide the month and year for each immunization. Starred (*) immunizations must be current
When you have finished all of that, please fill out the begining of ACA Health Form #2 (.pdf) and have a doctor fill out the rest.
Please note: When registering, you must select the grade that the camper will be going into for Fall 2012.
