Registration and Medical Forms
There are three portions to registering.
First, you must fill out the Online Registration, where you will enter some key information about the camper, select your program as well as pay a deposit of $200 per session and an non-refundable processing fee of $50.
Second, fill out the Online Health Form 1 (PDF) 2 MB.
PLEASE NOTE: Two of the first few questions on this form are "Dates will attend camp" and "Age on arrival at camp" both of which require you to scroll through the options and then highlight the desired selection. If you do not highlight the option is will not save.
Provide the month and year for each immunization. Immunizations must be current. This will require Adobe Reader. If you do not have Adobe Reader you can download it here. Check back shortly for this link. Please be advised, when you fill out the Online Registration form you can scroll through all of the sessions and dates and click all that apply to your camper. This also goes for the age of the child. Although the form options begin with four, you can scroll through and select other options.
Third, send us a copy of your child's Physical form from no earlier than June of 2012 or have your pediatrician fill out Health Form 2 (PDF) 2 MB and fax or email it to the camp within 10 days of registration. Children without immunizations and records will not be accepted into the camp.
Please note: When registering, you must select the grade that the camper will be going into for Fall 2013.

Sign up for one of the orientation nights. The Lower Camp Orientation is on 5/30/12. The Upper Camp Orientation is on 6/11/13.
